I confirm that if I wish to return my Livia device for a refund and it is not shipped to iPulse Medical in accordance with the terms of the refund agreement (i.e., the device must be shipped no more than 120 days after the date of purchase in the case of a one-time purchase, or between 4 months and 12 months from the date of initiation of a subscription), I will not be entitled a refund.
